影/奇美醫攜手基層院所建構安寧緩和醫療照護網絡

每年十月的第二個星期六是「世界安寧日」，2022年10月8日這一天也是奇美醫學中心奇恩病房成立第十四週年，為響應世界安寧日主題:「療癒心寧和社區」，院方舉辦音樂饗宴、義賣及藝廊油畫展一系列活動，攜手基層院所共同建構以病人為中心之社區安寧緩和醫療照護網絡，提升社區機構安寧療護專業知能，希望為病人與家屬帶來更好照護與醫療。

『70歲陳女士，罹患糖尿病多年；疫情期間不幸感染新冠肺炎(COVID-19)重症，引發呼吸衰竭與敗血性休克，在加護病房住了將近一個月，雖然幸運撿回一條命，但手指與腳趾因升壓劑導致發黑壞疽，醫生建議截肢保命，但陳女士不願意接受而拒絕，另一方面因肺纖維化也需長期氧氣治療，家人實無力在家照顧，因此被送到長照機構安置。由於之前插管住院的慘痛經驗，就算病情再惡化，陳女士也拒絕被插管、電擊或使用升壓劑，最後由家屬代簽拒絕急救意願書(無法寫字或蓋手印)，並申請奇美醫學中心安寧居家服務。
奇美醫學中心安寧居家服務團隊評估通過後立即提供無縫接軌服務，除了安排每週一至兩次至長照機構訪視，讓陳女士免於往返醫院的奔波困擾，減輕疾病帶來的痛苦，更在疫情期間降低交互感染風險，能安心在長照機構接受照護；服務團隊也適時提供家屬身心靈的諮詢與建設，陪伴末期病人及家屬面對死亡的各種調適，讓生死兩無憾！』

奇美醫學中心緩和醫療中心主任柯獻欽表示：2008年7月，奇美醫學中心的「奇恩病房」在許文龍創辦人、董事會、邱仲慶榮譽院長及院內諸多同事的推動下誕生。經過七年的努力，安寧照顧團隊由「奇恩病房」的任務編組提升到2012年5月成立的安寧療護委員會，做為整合相關資源，提升同仁安寧照護的態度與知能，提升病人生命末期照護的品質。

2013年8月成立「緩和醫療中心」，其中包含癌症、非癌症、急重症與教育研究各組，不侷限於服務癌末病人，期望各類疾病的末期病人都得以善終，並致力於安寧緩和醫學的教育與推廣，希望各類專科醫師都能有安寧療護的觀念，讓末期病人無論在那個專科均能得到安寧療護。並配合衛福部政策推廣安寧緩和醫療社區照護模式(每個月長照機構安寧居家服務人次約200人次)，積極與基層院所合作(目前已合作五家)，讓在地長照機構、醫療基層診所、居家護理所提供居家安寧療護服務，共同建構以病人為中心之社區安寧緩和醫療照護網絡。
