確保血糖穩定 「連續性血糖監測器」精準控糖更有感！

科技助益精準控糖，生活型態調整事半功倍，但仍提醒，想要穩定控制血糖，還是需靠長期生活型態(飲食、運動等)的調整才是最重要的關鍵
臺灣妊娠糖尿病從原本盛行率5至7%，因高齡產婦的增加和篩檢技術的進步，逐步上升至12至15%。而高血糖狀況下發育的胎兒，生長速度可能會過快，可能因此增加成為巨嬰甚至胎死腹中的風險。孕婦可能會有羊水過多，早產及子癲前症等情況，並且增加剖腹產的機會。新生兒出生，則可能發生低血糖、低血鈣以及呼吸窘迫等狀況。小孩未來體重過重、代謝症候群與得到糖尿病的機會也都會增加。奇美醫院建議裝設「連續性血糖監測器」，可隨時關注血糖值變化，精準控糖更有感！

一名38歲陳太太多年求子，經由不孕症治療，終於懷上第一胎，但在第20至24週進行空腹75克血糖檢測時，卻發現空腹血糖156mg/dl(正常值為60至95mg/dl)，一小時血糖355mg/dl(正常值為60至140mg/dl)，二小時血糖278mg/dl(正常值為60至120mg/dl)。自行上網搜尋發現有可能是妊娠糖尿病，不僅會增加子癲前症的風險，也可能造成巨嬰，甚至新生兒低血糖，進而影響智力，感到憂心忡忡，於是至奇美醫院內分泌新陳代謝科主治醫師黃偉輔門診諮詢。
黃醫師診察後，建議陳太太先控制好血糖，一天至少要監測一次早上空腹血糖以及三次飯後一小時的血糖，將風險降低，以保護孕婦自己與胎兒。陳太太遵從醫師醫囑，認真控制飲食與運動，每天扎手指頭四、五次，導致每根手指頭都是洞，疼痛難耐，遂向黃醫師反映監測血糖的問題，黃醫師建議裝設「連續性血糖監測器」，可隨時關注血糖值變化，總算讓原本疼痛不已的手指得到解脫。後續定期回診有效控制血糖，最後平安生下3,200克健康女寶寶。

奇美醫療財團法人奇美醫院內分泌新陳代謝科主治醫師黃偉輔指出，多數妊娠糖尿病媽媽大多數(90%以上)都可以藉由飲食控制與密切的血糖監測，得到良好的血糖控制。根據2018年妊娠糖尿病照護手冊指出，孕婦利用血糖機每日自我監測血糖四次以上(例如：一次空腹加上三餐餐後一小時或二小時)。建議目標為空腹≤95 mg/dl，餐後一小時≤140 mg/dl，餐後二小時≤120 mg/dl。
連續血糖監測(CGM)的細針前端有一個葡萄糖感應器埋入皮下組織，可以每隔幾分鐘測量皮下組織液中的葡萄糖，並將數據傳送到接收器或智能手機上，讓使用者隨時查看自己的血糖曲線和趨勢
臺灣已上市血糖管理新科技－連續血糖監測(Continuous Glucose Monitoring，CGM)，可以了解一天24小時的血糖波動與數據。連續血糖監測器是在糖媽咪身上安裝一台與50元硬幣大小的傳感器(俗稱：小貝殼)，儀器的探頭回植入皮下組織中，附著到組織液中，藉以測量一整天的葡萄糖變化，並用線圖的方式表現在手機的APP上，讓媽咪隨時可以知道目前血糖的數值與波動的情況，進而即時調整飲食、運動和用藥。

黃偉輔醫師說明，整體而言，連續血糖監測器不僅可以降低傳統扎手指量測血糖的次數，亦可以立即了解自己適合吃什麼食物，食物份量是否太多，隨時調整，孕媽咪可以聰明自在的選擇適合的食物，讓懷孕期間的血糖，既穩定又安心。
臺灣於112年11月01日起針對連續血糖監測(CGM)健保給付有新規定：(1)第1型糖友，在特定情況下（如血糖波動範圍大、反覆低血糖，懷孕）(2)懷孕之第二型糖尿病或妊娠糖尿病，且接受胰島素注射者，皆可以透過醫院申請健保支付。
