圓禿作祟 國中女鬼剃頭 嚴重掉髮剩一成髮量

前陣子台南市一名國中女生因短時間快速掉髮至安南醫院就診，經皮膚科陳郁蓁醫師檢查後發現頭皮有多處非疤痕性落髮，經抽血檢查後排除感染問題，確定診斷為圓禿。決定接受門診類固醇脈衝治療。此外，抽血檢查也發現病人有缺鐵性貧血及鋅不足情況，因此陳醫師也加入鐵劑跟鋅片來補充。治療期間雖國中女生頭髮曾一度掉到只剩十分之一，但其仍展現無畏勇氣持續規則地完成12週治療療程，療程結束後頭髮也如預期中的順利恢復原貌，病人跟媽媽總算放下心中大石頭。
陳郁蓁醫師指出，圓禿也俗稱「鬼剃頭」，是一種自體免疫性疾病。病程通常會反覆發作，大約有75%患者一輩子會發生不只一次。圓禿在任何年紀都可能發生，但以年輕人更為常見且沒有好發之性別差異，有些病人可能同時會罹患自體免疫性疾病，常見相關疾病如白斑跟甲狀腺疾病。圓禿初期臨床表現可能只會有局部數個位置呈現圓形禿髮但不痛不癢，很多患者都是經由同儕或家人告知才至醫院就診，嚴重時甚至可能會影響全身各處的毛髮比如：眉毛、睫毛、鬍子、腋毛或私密處毛髮都有可能。雖有些病人有時不經治療就能自癒，但陳郁蓁醫師仍建議還是要諮詢皮膚專科醫師，以爭取治療黃金時期。現行已有多種圓禿治療方法可供選擇，包括類固醇藥膏、局部類固醇注射、其他局部、全身性免疫抑制劑及口服小分子藥物等，皮膚科醫師會依照患者年紀與落髮面積給予最適當建議與治療。
陳郁蓁醫師表示，圓禿所造成的短時間快速落髮，因會影響病人外觀進而一定會對心理層面造成莫大壓力影響正常社交生活，不可小覷不治療對於自身諸多層面之影響。
