增生注射併物理治療解肩頸酸痛 奇美醫院呼籲：改善不良生活型態 防止疾病找上門

在現今繁忙的工商社會，凡事講求快速、效率，這樣的壓力導致許多人全身肌肉長期緊繃僵硬，尤其無時無刻的使用電腦、智慧型手機等電子產品，經常維持著低頭或脖子前傾等不良姿勢，更加重肌肉僵硬的問題。
案例：40歲陳先生，是一位科技公司的工程師，三個月前開始出肩頸痠痛，有時會伴隨雙手麻木、頭痛、上背或下背痛的狀況。過去，他也曾出現類似的狀況，但多數時間休息之後得以改善，但這次的症狀持續了一陣子，因此來奇美醫療財團法人奇美醫院復健部主治醫師程信翰門診求助。經過病史詢問、工作狀況了解、鑑別診斷區別發現為頸椎小面關節炎，採用物理治療、增生注射及衛教後，痠痛的問題終於獲得解決，長年嚴重失眠也明顯獲得改善。
奇美醫療財團法人奇美醫院復健部主治醫師程信翰說明：近年來，因為生活習慣、工作型態改變以及長時間使用手機、電腦等電子產品，導致肩頸痠痛的病人逐漸增加，而工作相關的肌肉骨骼痠痛也非常常見，如果不加以治療，可能會影響到日常生活，甚至影響工作效率。根據勞動部勞工保險局2022年勞工保險職業病傷病給付資料顯示，手臂頸肩疾病人數為311位，其中以40至59歲占最多。根據一篇發表於國外醫學期刊的研究統計。
主講者復健部主治醫師程信翰分享肩頸痠痛治療成果。
醫師透過病史的詢問，了解疼痛發作的情況、疼痛持續的時間及特徵(例如，是否延伸到上肢或是否伴隨感覺異常)以及限制活動的程度。理學檢查，包括觀察頸部活動、關節活動範圍、肩頸肌肉的觸診…等。實驗室檢查，包含X光、骨骼肌肉超音波、電腦斷層、核磁共振及神經傳導檢查。總和上述結果，才能精確診斷病因，以利後續治療。
奇美醫療財團法人奇美醫院復健部主治醫師程信翰指出：治療肩頸症候群，重點還是要針對確認的病因才會有效。舉例來說，痠痛藥膏能舒緩肌肉拉傷，但對於椎間盤的疼痛效果不佳。口服止痛藥，只能短暫緩解部分神經根壓迫的不適，但藥效過了，症狀又會再次發作。案例陳先生的狀況中，藥物、推拿、按摩、針灸及物理治療都必須反覆執行才能舒緩症狀，所花費的時間較多也較容易影響工作。因此，與我討論後決定接受增生注射，一來澈底處理神經壓迫，二來也較節省原本其他治療所需的時間。完成療程後，痠痛的問題終於獲得解決，長年嚴重失眠也明顯獲得改善。
根據奇美醫療財團法人奇美醫院復健部統計近五年(2017年至2022年)因肩頸症候群至求診人數約2,000位，其中肌肉骨骼732位(38%)、神經問題1,194位(62%)，年齡大多30歲至65歲的中壯年族群。2022年與2021年相比，人數成長近2成，尤其為30歲至50歲，顯示疫情下居家辦公及運動量減少，導致肌肉過度使用或耐力不足等狀況。
長久來看，要避免工作相關的肌肉骨骼痠痛，還是有賴生活型態及工作習慣的調整。這些習慣與知識，往往來自復健醫學與職業醫學領域的前沿研究與經驗，需要透過多方的管道才能更快的普及。不僅是傳統上醫師透過門診衛教醫療資訊，還包含了法規要求一定規模的公司執行臨場服務，辦理勞工健康服務事項，以期快速提升民眾與勞工的健康知識。
