安南醫院傳授「職業病」判斷準則 泥水師傅罹旋轉肌袖症候群 職醫診斷助減經濟負擔

安南醫院職業醫學科主任陳證中解說旋轉肌袖症候群。

勞動工作者最常見的困擾，便是難以判斷自身病痛是否源於工作，因而錯失申請補助機會。安南醫院職業醫學科主任陳證中分享近期案例，58歲陳先生左肩疼痛及左手抬舉困難約三個月，期間接受幾次治療仍難以改善，故暫停工作休養。直到他疼痛難耐至安南醫院就診，經檢查發現「倒罐測試」與「離背測試」皆陽性，且超音波顯示左肩棘上肌肌腱部分撕裂，被診斷為左肩旋轉肌袖症候群，因疑似與其職業相關，進而轉至職業醫學科評估。

陳先生原為一名年資約35年的泥水師傅，每天工作8小時、每星期工作6天，有鑑於工作時須手持抹刀、推刀、泥盤、抹平工具等，加上雙手進行泥水施工與紅磚堆砌，須反覆抬舉過肩，皆促進旋轉肌袖症候群發生機率。因此，前有陳先生泥水工作年資，後有左肩旋轉肌袖症候群產生，符合先有工作暴露後致病之合理時序性。

此外，根據陳先生自述，過去無左肩外傷或骨折等病史，平時亦無從事需雙手反覆抬舉過肩之休閒活動或運動，而職業相關旋轉肌袖症候群確為臺灣勞工職業災害保險職業病種類表之一，故經安南醫院職業醫學專科醫師抽絲剝繭後，評估其疾病與職業具有因果關係，且罹患之左肩旋轉肌袖症候群與職業相關，此案為職業疾病無誤。

陳證中主任表示在安南醫院職業醫學科門診協助開立診斷書後，協助至勞保局申請職業疾病認定，順利通過審核為職業疾病。

陳證中主任提及後續，在安南醫院職業醫學科門診協助開立診斷書後，陳先生持診斷書與完整資料至勞保局申請職業疾病認定，順利通過審核為職業疾病，協助他在停工無薪期間，可領取勞保傷病給付之補助款，同時享有醫療給付減免等，大大減輕其經濟壓力。
